Bengaluru: The share of registered births classified as ‘institutional’ in Karnataka has fallen from 94.5% in 2021 to 86.3% in 2025, with the decline substantially sharper in rural areas, according to the state’s annual reports on registration of births and deaths.

The rural share fell from 80.2% in 2021 to 53.2% in 2025, a decline of 27 percentage points. In urban areas, the corresponding fall was from 97.4% to 93%, or 4.4 percentage points. The share stood at 77.7% in 2022 and 76.6% in 2023 before dropping to 68.3% in 2024 and 53.2% in 2025. Urban institutional births remained above 97% through 2024, before falling to 93% in 2025. The overall share had remained largely unchanged between 2021 and 2023 — 94.5%, 94.6% and 94.9%, respectively — before falling to 92.7% in 2024 and 86.3% in 2025.

Institutional births are part of its ‘Type of Attention at Delivery’ classification, alongside doctor/nurse/trained midwife, traditional birth attendant and relatives/others, according to the report. Institutional deaths are similarly a classification of attention at death. The decline has been gradual in rural areas but became particularly pronounced in the past two years.

It increased from nearly 8.5 lakh in 2021 to nearly 8.8 lakh in 2025. The 2025 figures also show that the rural-urban difference is now much wider. Of more than 1.7 lakh births registered in the rural sector, 90,927 were classified as institutional. In urban areas, nearly 7.9 lakh of nearly 8.5 lakh registered births were institutional. The share of registered deaths classified as institutional increased from 30.1% in 2021 to 33.5% in 2025. In urban areas, it rose from 54.4% to 62.9%, while the rural share remained below 10% throughout the five years, at 7.8% in 2025. Because total registered births rose from more than 8.9 lakh to nearly 10.2 lakh during the period, the share fell.
