The recipient’s baseline anti-A antibody titre posed a risk of antibody-mediated rejection. The hospital initially planned about six plasma exchange sessions to reduce antibody levels. However, the patient developed lower-limb cellulitis after the first session, prompting doctors to postpone the transplant for about a month until the infection had resolved.

After his recovery, the team used sequential antigen-specific immunoadsorption treatments to reduce the anti-A antibody titre to 1:8 before proceeding with the transplant.
